NOTICE REQUEST FOR QUALIFICATIONS The City of Huntington Public Works Department requests letters of interest from qualified professinal firms for engineering, labor, equipment and materials for the design of construction documents concerning slippage along Honeysuckle Lane approximately 200 yards ''up the hill'' off of Fifth Street Road. Proposals will be received until 2:00 p.m., local prevailing time on August 23, 2016, at City Hall, Room 16, located at 800 5th Avenue, Huntington, WV. Each proposal must include the following: 1. Firm name, address, phone number, fax number, email address. 2. Names and resumes listing the technical qualifcatin of the persons who will perform these services. 3. Description of experience in related work. 4. Reference list of clients whom the entity has represented in similar projects, with names and current telephone numbers of contracts. 5. Four copies of the proposals are to be submitted. The following criteria will be used in the selection process: 1. Project Manager/Engineer. (20 points max) 2, Strength/Experience of Assigned Staff including Subconsultants. (15 points max) 3. Familiarity with the project. (30 points max) 4.
