NOTICE OF HEARING TO: Jami Ann Triplett 2610 Orchard Ave. Huntington, WV 25704 In the Matter of: Jami Ann Triplett, RN On July 21, 2016, the Ohio Board of Nursing issued a Notice of Opportunity for Hearing (Notice) to Ms. Triplett, mailed on July 22, 2016, via certified mail, to her last known address of record, 2610 Orchard Ave., Huntington, WV 25704. The mailing was returned marked "Unclaimed." The Notice states that the Board intends to consider disciplinary action against Ms. Triplett's RN license based on her admission that she self-administered opiates without a legal, valid prescription and reported to work while impaired; her false statement on her application for a nursing license; and her omission of prior employment on an application for employment. The foregoing would constitute grounds for disciplinary action pursuant to 4723.28(A), (B)(8), (B)(10) and (B)(16), ORC, and Rule 4723-4-06-(P), OAC. Ms. Triplett is entitled to a hearing in this matter if it is requested within 30 days of the last date of publication of this Notice. She may appear at hearing in person, by her attorney or by another representative permitted to practice before the Board, or she may present her position, arguments or contentions in writing.
