NOTICE OF APPLICATION On January 8, 2016, Greene's Transportation, LLC filed an application, which was subsequently amended, for a permit authorizing; To operate as a contract carrier by motor vehicle in the transportation of passengers in limousine service, through the use of specialized limousines that provide specialized multi-passenger van service, to and from the offices of physicians and of other providers of health care and medical services - such transportation being restricted to those passengers who will receive, or who have received, medical treatment and services at such facilities - between points and places in Putnam and Cabell counties, on the one hand, and points and places in West Virginia, on the other hand, under contract with Medical Transportation Management, Inc. Rates: Curb to Curb Loaded Business Hour Rates per One Way trip: 0-10 miles $12.00 Ambulatory $17.00 wheelchair 10 miles to endless miles $1.05 per mile Ambulatory $1.10 per mile wheelchair Curb to Curb Loaded After Hour Rates per one Way trip: 0-10 miles $14.00 Ambulatory $19.00 wheelchair 10 miles to endless miles $1.10 per mile Ambulatory $1.15 per mile wheelchair Additional Charles: Wait Time $0.20 per minute Unloaded Mileage $0.70 per mile Anyone desiring to protest or intervene should file a written protest or notice of intervention within ten days following the date of this publication unless otherwise modified by Commission order.
